Office Policies and ProceduresWe value each and every patient and will strive to take care of your foot and ankle problems to the best of our ability. Referrals:For patients covered under managed care, you are responsible for obtaining a valid referral from your primary care doctor. If you are unsure of your policy requirements, please ask the front desk for assistance. Please contact your insurance company to make sure that Dr. Graham is a preferred provider for your plan. Otherwise, your benefits may be reduced or eliminated by your carrier. Payments:Your insurance plan requires you to pay a co-pay amount for each and every face to face visit with Dr. Graham. Please check your insurance card for the correct amount. This co-pay will be collected prior to your appointment. All unmet deductible amounts must also be paid unless prior arrangements are made. Should you be unable to pay the balance owed on your account, please speak to the front desk about signing a financial agreement. The minimum payment is $50 and credit may be extended for a maximum of four months. Your first payment must be paid when the agreement is signed. Scheduled payments must be paid by the due date or a $20 rebilling charge will be applied, the contract will no longer be valid and the payment will be due in full. We dispense many comfort items that are non-covered by insurance plans such as; over the counter inserts, air casts, ankle wraps, bunion splints, silicone toe shields, pads, etc... These items must be paid for at the time they are dispensed and are not billable to your insurance plan. Form Completion / Record Copies:Due to the overwhelming paperwork that Dr. Graham is asked to fill out, all disability, loan repayment forms etc... will require a $12.00 payment, prior to completion. Please allow three business days for completion. Should you need records copied, the fees are: $2.67 each page for the first ten pages, .55 cents for each additional page (11-50) and .22 cents for pages 51and higher. These fees may vary if the request is made by someone other than the patient or the patient's personal representative. X-ray copying charges are $7.50 for each x-ray or $3.00 for a digital copy. X-rays over a year old should probably be retaken rather than copied. Please allow three business days for completion. Fees subject to change, contact office for current fee schedule. Address/Information Changes:Please: Notify the office if you have a new address, phone number, insurance card or primary care physician. Returned Checks:There will be a $25.00 fee charged for returned checks. Emergencies:The office recording will give you an emergency number for after hour contact. During the unusual ice storm/power outage, a few winters ago, we were unable to reach many patients to notify them that the office would be closed. A cell phone number and e-mail address would help alleviate that problem, should it happen again. Please give this information to the front desk personnel and they will update your account.
